Hi.  Thank you for your question.  As my other colleagues suggest, for your desired results you will need surgery as fillers will not refine your tip.  You should schedule an appointment with an experienced Board Certified surgeon for a consultation.  Best of luck to you.

Definitely consult with your surgeon.  If an implant was used , it would not be good to pierce the implant since it is usually in the vicinity of the anterior membranous septum. 

 Poor skincare can affect the skin overlying the nose, but will not affect the surgical results from the rhinoplasty procedure which involves sculpting both bone and cartilage. For skincare issues, best to seek care by a dermatologist.
