In my experience, an enlarged (dilated, patulous) pore(s) can be narrowed or closed entirely by inserting a very fine, insulated electrode that transmits a rapid, short radiofrequency current that generates just enough heat to irritate the lining of the pore, which then tightens or closes entirely upon healing. The insulation of the electrode probe limits damage to the epidermis and permits only wounding of the inside of the pore below the skin surface. This ensures closure without engendering significant risk of surface damage and scarring. Several dilated pores, when present, can actually be treated in this fashion in literally just a couple of minutes. Local anesthesia instilled under each pore makes the process virtually painless. (When numerous dilated pores are present, I have found fractional microneedling to be a more practical approach to treatment.)
In my hands punch excision and suturing is the best treatment for a large pore If you have extremely oily skin, I may recommend two weeks or isotretinoin (Accutane) before and after the surgery to decrease the oil before surgery and enhance the healing and prevent oil separation of the sutured excision post operatively.
