38 year old female that has had filler and never felt great about the protruding lip. I believe my philtrum is too long. Any advice is appreciated.
Answer: Eccentric lip lift You are an excellent candidate for an eccentric lip lift. An eccentric lip lift does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners. This takes 1 hour to perform under local anesthesia and costs $4,000. Best Wishes, Gary Horndeski, M.D.

Answer: Candidate for a Lip Lift? I would say "Yes" Based on the photo provided, but subject in physical examination, you appear to be a candidate for a Bullhorn Lip Lift. I would first dissolve all filler in your lips, which currently distorts a more complete evaluation. The length of the "white lip" and likely receding "red" lip make this a great solution for increased red lip show and smaller vertical lip height. Your anatomy makes lip filler less than ideal for a variety of reasons.

Answer: Lip Lift Candidacy – Addressing a Long Philtrum and Lip Proportion Hi there, and thank you for sharing your concern, you’re definitely not alone in feeling unsure after filler, especially when it doesn’t achieve the balanced aesthetic you were hoping for. Based on your photo and your description, it does appear that you may have a slightly elongated philtrum and some disproportion between the upper and lower lips. This can cause the upper lip to look less defined or less youthful, even with filler. A lip lift — particularly a subnasal bullhorn lift — could be a great option for someone like you, especially at age 38 when natural collagen support begins to decline. Here’s what a lip lift can help with: Shortening the philtrum, creating better facial balance Enhancing the vermilion show (the pink part of your lip) without the “overfilled” look Lifting and shaping the upper lip in a way that filler often cannot achieve on its own Are you a good candidate? From what you’ve shared, yes — you likely are! Ideal candidates typically have: A longer philtrum (often over 13–15 mm) Good general health Realistic expectations and a desire for a more permanent, refined enhancement rather than volume alone Of course, final recommendations should be made after an in-person evaluation where measurements can be taken and your anatomy reviewed more closely. I’d be happy to evaluate you in more detail and discuss the benefits, risks, and healing process if you're considering this route.
