Hello! Most of the time, patients will complain about drooping or excessive flare when smiling. Some of that is inherent to the structure of the face and smile muscle strength. But, there are things that can be done. A caudal septal extension graft at the tip provides an anchor point to prevent excessive movement of the tip. If the nostrils are too wide, an alar base reduction can create narrowness and develop scar tissue which will resist some of the untoward effects of flaring when smiling. Hope this helps!

The alar base refers to the entire width of the base of the nose when viewed from below. The area between the nostrils is called the columella. Alar base reduction narrows the whole width and makes the nostrils smaller but does not affect the columella.
