I'm 27 years old and I would like a lift in my skin as shown in the pictures as I am starting to get triangle shaped bags and sunken eyes which I can see in bad lighting. What procedure would be best to achieve this? I was thinking a canthopexy and endoscopic brow lift. Think I'm too young for a mid face lift? I was thinking first to get PDO threads to see if I would suit it before going down the route of surgery. Wondering if a lower blepharoplasty would be a good choice too?

In terms of non-surgical treatment options, I would recommend a Chemical Brow Lift (using Botox, Dysport, Xeomin, or Jeuveau) and filler injections to rejuvenate the temples, eyebrow, eyelid, eye troughs, upper eye hollows, and forehead. Well that can be easily accomplished with the use of an appropriately injected robust HA filler. I do that for people practically every day in my practice. The filler can not only help to reestablish a more feminine, heart-shaped oval face, but (much like what you are doing with your finger) will smooth the under-eye regions and create a more gradual transition between the upper mid-cheek and the under-eye regions. I gave up threads several years ago, since the complication rate of dimples, divets, thread extrusion and infection proved unacceptably high, the procedure was expensive and engendered significant post-treatment downtime. And no need, certainly not at this stage and from what can be seen in the photos, for expensive, aggressive, invasive surgeries. You would be wise to consult with an experienced, board certified cosmetic dermatologist filler expert. Filler teatment for this, in experienced hands, should take no more than about fifteen to twenty minutes with immediate, on-the-spot results and engender little to no significant downtime. Answer: Midface You're description suggests a midface vertical lift will address two issues. The descent of the cheek is causing the lower eyelid to lengthen when the cheek fat falls below the orbit bone. The vertical lift that I do lifts the entire cheek vertically and the fat is lifted up over the orbit bone edge and over the cheek bone. The lift also addresses the "tear trough" area. The lower eyelid is done at the same time.
