Overprojected Graft - how can this be fixed? (Photos)

Question

I had Septorhinoplasty last year. This was to fix a deviated septum. Surgeon said issue mid surgery and he had to put a stunt graft on the columella.The result is an overprojected tip( I had tiny button tip pre this surgery ) . The graft is also causing issues when I smile - it catches and curves up around the graft now. My nostrils are also more on show.

Based on the photos, you are a great candidate for revision rhinoplasty. It looks like too much cartilage was taken from the nasal sidewalls which pulls the nostrils up some. This can be improved with alar grafts to help push the nostril rim down. The columella looks very stiff and hard which can be softened as well. Be sure to see a rhinoplasty expert for consultation.

Hello and thank you for your question. Based on your photographs, you are a great candidate for a revision rhinoplasty. Maneuvers for your surgery would include correction of hanging columella , tip refinement, nasal tip deprojection, tip elevation, among other maneuvers. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.
