In general, buccal fat removal is for fullness in the cheek to give you more contour; from your photo, I would not recommend it since you already have a defined cheek area, and removing the buccal fat will not benefit you.On the other hand, a mini facelift that addresses the mild jowling area might help if performed by an expert. Also, if the issue is mainly the turned-down angle of the mouth, a simple injection of Botox in the muscle responsible for that action might be enough.
