Question

What nose refinements do I need and what is the price range for those changes? I understand that skimping on an experienced surgeon is a bad idea. Rather I'd like to know an estimate before consult (if possible).

Based on your photos, you appear to have a relatively broad and flat and wide nasal appearance, with nostril asymmetry and a retracted columella. Depending on your examination, rhinoplasty surgery may be considered for achieving narrowing and straightening of your bridge tip and nostrils on front view, along with profile refinements. You have two options. One simple option would be t just narrow the nostrils which takes less than an hour under local anesthesia. If you want your entire nose narrowed a full rhinoplasty including breaking the bones to narrow them would be required

"Need" is always a difficult word in the plastic surgery world as no one NEEDS cosmetic surgery. That being said, if you are bothered by the width of your nose, some things you could consider would be fracturing in of the nasal bones, alarplasty to get the base narrower, and some tip cartilage modification to get the tip narrower too. If there are other things you don't like about your nose, that might change how much needs to be done. You also have thick skin, which will be alimiting factor in how small and defined you can get your nose. Nonetheless, I feel that this is a very typical rhinoplasty and you can look up what are common prices in your area.

What could a rhinoplasty do for you? based on the views that you show, you appear to have a wide or “bulbous“ nasal tip, a wide bridge, a wide nostril base, and perhaps a retruded columella. The wide bridge tip and nostrils could be addressed with a standard rhinoplasty including alar plasty. and if the retruded columella were to be addressed, this would require harvesting septal cartilage to be used as a graft to project the columella a couple of millimeters downward. Adding this to the standard rhinoplasty will make the procedure somewhat longer and more expensive. You will find that price varies dramatically from one surgeon to the next. You will generally find that prices are higher for surgeons who specialize in doing only nasal surgery. Most patients find that the more expensive cost is well worth it, since rhinoplasty has evolved into a specialty unto itself, and requires a great deal of experience, particularly with ethnic noses. Your skin on the tip of the nose does appear rather thick,, And may need follow up Cortizone injections to reduce swelling, scar tissue, and refine the skin to produce more definition in the tip area.

A closed rhinoplasty approach can accomplish narrowing your nasal tip and nasal bridge line. All of the incisions are placed on the inside of the nose. An alar plasty is performed to narrow wide nostrils at rest.  It's also possible to release the lateral ligaments to reduce the dynamic flare and widening when you smile.
