Based on this photo, it appears that lowering of nose dorsum would help. The addition of a chin implant will definitely improve the balance of your profile. Both of these procedures can be done in the same surgery. Good luck.
From the one limited photograph, you have an overly projecting nose and an under projecting chin. A closed rhinoplasty revision can accomplish shaving down the residual dorsal hump, and slightly de-projecting the nose. Placement of a chin implant would accomplish Augmenting your chin forward for better facial balance and proportions, especially with respect to the projection of your nose from the side profile. A revision rhinoplasty is performed under general anesthesia, while placement of a chin implant can be performed under local anesthesia.
There are a lot of variables to take in the consideration when doing a facial assessment. If somebody’s had previous surgery that ideally, we should see before and after pictures. Chin augmention can be helpful if the chin is weak to give a more balanced appearance, especially if the nose looks large and projected.I suggest having a few in person, consultations with local plastic surgeons or oral surgeons. There is some discretion in regards to where the chin implant should be placed, especially in regards to creating vertical versus horizontal projection. Care needs to be taken to make sure the implant does not encroach on the mental nerve foramen. Best, Mats Hagstrom, MD
