It appears to me from your photos that you have prominent buccal fat pads. Each person is born with buccal fat pads. In the beginning, it takes time to understand the anatomy and location of the buccal fat pads. I would get another opinion form a surgeon that is very experienced with this procedure.

It is likely the fat was just "hiding" that day. In 25 years I've seen only one person where I couldn't find the fat the first time (and had to return) on one side.
It is likely that it can be removed still.
We do this in the office with local anesthesia.

Hello, hope you are well. Generally speaking, everyone has buccal fat. Like anything else, the quantity and anatomy of the fat compartment varies somewhat from person to person, but it's there. Even for someone with a relatively smaller fat compartment, the quantity would be measured in cubic centimeters ('cc') and not on a granular scale. Having said that, the procedure has its challenges and sometimes a second attempt is necessary.

Based on the series of posted photos I see buccal fat pads. But in my practice if I am having difficult time in locating the buccal fact compartment i use in surgery ultrasound to assist. Seems you were not given this option. Only doing this revision under ultrasound guided location can achieve desired removal. Fee $8,000. Best to virtual consult with...
