I am afraid better photos needed (all profiles and obliques, also underneath nostrils), however this indent might not be such but a cyst at the alar base due to a suture that your tissues are trying to expell out.

Based on what we can see, your alarplasty scar on the right side is a bit distracting. Depending on your examination, this would likely require an in-office scar revision type procedure that may not necessitate general anesthetic. You had your nostrils reduced and that is likely a scar from that part. We can do a scar reduction / improvement in the office likely to help with that.

It looks like tissue was removed in an inappropriate location during your procedure.
I have no idea why a plastic surgeon would make the cut in that location.
I think this is going to be very difficult to improve it.
I don’t think non-surgical options are going to be helpful and I think surgical options are going to be challenging as well.
I wish I could be more optimistic, and maybe there are other plastic surgeons out there who feel more optimistic about the ability to improve this.
If tissue was inappropriately removed, then correcting the problem needs to be based on restoring that tissue, which is very difficult to based on the shape in the area.
To start with you should get a complete collection of before and after pictures.
If you don’t have before, and after pictures to ask your surgeon to forward the pictures they took, and
Maybe the surgeon never took after pictures if you didn’t follow up with them?
I generally do not recommend people travel long distances for elective surgical procedures.
Surgery is best done locally.
