Is there a successful procedure to correct fat transfer that ended in cyst formation on forehead? (Photo)

I had fat transfer done 4 years ago to my forehead as had indentation due to ken along injection to treat acne. It worked initially then formed a bump which the surgeon treated with kenalog. But later needed restalyn. The surgeon's partner thinks there is a cyst and wants to make an incision to excise the cyst on my forehead and I am really nervous. Not sure I should do it as don't want a worse outcome.

I suggest getting an ultrasound or MRI to see if there is a cyst that has formed or if it is just a reaction to the fat.  Regardless, the only options include injections to decrease the size, mini-liposuction or even possibly subcision.  I have used radiofrequency devices and co2 laser in the past with good results.  I suggest getting a second opinion from a cosmetic dermatologist with experience doing fat injections and fillers.  Best, Dr. Emer.

Fat grafting is great when done properly. However, even the smallest amount of fat placed in the wrong area can have devastating consequences. It is for this reason that finding a physician with focused skills and training in fat grafting is of tantamount importance.The best treatment for the removal of excess or poorly placed fat is microliposuction. This is a precise removal of fat using specialized lubricious microcannulas. The cannulas I use are typically smaller than the needle used to draw blood.

As you unfortunately discovered, fat grafting to the forehead can be unpredictable due to the relative paucity of fat under the skin and the hard bone beneath. This can result in lumps if too much is placed in one spot. Fat grafting is a great modality for correcting and augmenting certain areas otherwise. At this point, I agree with your surgeons partner, take out the lump through a small incision and allow it to heal. If a contour defect is left afterwards, correct with very small amounts of filler instead. Good luck.
