In your after photo, you can see your nipples are pointing outward. The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, a small amount of fat transfer can be performed.

You will have some skin laxity following explantation but your age and photos suggest that you should have subsequent contraction of the tissues and may ultimately have an acceptable appearance. I would personally recommend delaying any fat grafting until the breasts have fully healed and matured. If you are unhappy, fat grafting could be done at a later date. There is a good chance you will find this unnecessary.

Once implants are removed, there will likely be flattening and drooping that can be corrected in large part with a breast lift. As far as the volume that is attained, that is a bit difficult to predict given the period of time that you have had the implants thinning the remaining tissue. No one can truly predict this as each soft tissue envelope will respond differently, but you may do well with just removal. If there is any doubt, explant using the original incision, followed by a period of several months of observation. After 6 months or so a decision for a breast lift with or without fat transfer can be made.

Thank you for sharing your question. Based on your age and apparent good skin quality I would expect you to do well with an explant alone. there are additional aesthetic risks depending on amount of capsule removal, if any, so be sure to have a discussion with your surgeon.

I would recommend a complete capsulectomy and explant, and I would place a drain on each side for five days so that no fluid accumulates in the pockets. Then I would have you wear a good compression bra 24/7 for 12 weeks, and it's amazing how the skin can "shrink back down" to your old shape without sagging. With this technique I rarely ever have to perform a lift when explanting, and in those cases I can almost always predict that beforehand. Also you gained some weight so you may have larger breasts than before. I would not recommend fat grafting- it just doesn't work very well!

it is hard to tell for sure without an examination. Generally speaking, fat transfer is a great option to restore some of the lost volume. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

An examination would tell how much more breast tissue you have now. You might want to consider going down in size with implants.

It is difficult to say how much native tissue you have without an examination, but you appear to have good skin elasticity. One option is to remove the implants and allow them to fully heal. If you are happy then you have avoided further surgery. If you are unhappy you can return for fat grafting, a lift, or both depending on your aesthetic goals.
