I have large glands - how can I permanently enhance my jawline? (photos)

Question

I had Kybella which accentuated my glands... Thankfully some fat returned and covered them back up. Filler created a really nice jawline without stretching the skin too strongly over the glands. I don't want to remove the glands. Is there another more lasting procedure than fillers that defines the jaw and lifts the submental fullness without bringing out the glands? eg. small facelift with incision in the temples? As a 6.0 ft woman I also want to avoid a too masculine jaw via implants. Tx

You are an absolute candidate for open neck deep plane lift, with the purpose to treat the underplatysma bulging: trimming only the bulging part of the gland, it is never a total removal, removing a portion of the digastric muscle, trimming the subplatysma fat which obviously can not be reached with kybela or any other external device. And if it is not necessary no incision behind the ears. 
Impossible that a high incision would solve the under jawline issues. 
My recommendation is to avoid creating more and more scar tissue which will make the surgery more difficult and farther to great results.

It is possible to do a partial removal of the portion of your salivary gland which is protruding. This, in addition to some other steps, can help to decrease the prominence of your salivary gland, and help sharpen and define your jawline. Not all surgeons perform this type of surgery, so you should do your due diligence and find one who has extensive experience in this. Best wishes.

You may want to consider liposuction of the neck. The swelling is possibly residual fat as opposed to salivary glands.  Kybella doesn’t always get all the fat dissolved
