What would you recommend for receding chin and overjet/overbite, sliding genioplasty and lipo? (photos)

The bite discrepancy will not be fixed with a chin surgery, but I think that a chin implant with chin liposuction can produce a nice change with the least amount of invasiveness.Kenneth Hughes, MDLos Angeles, CA

Short of orthodontics and orthognathic surgery, direct chin augmentation with neck liposuction would be beneficial. Whether the chin should be augmented by an implant or a sliding genioplasty is a matter for discussion. But with some overjet (meaning the vertical length of the chin is a bit short) and some neck fat, I would lean towards the slding genioplasty as it does a better job of pulling some neck structures forward that an implant does not do.

The photographs demonstrate a weak chin profile and fatty deposits in the neck. To augment the receding chin, a chin implant is placed through sub mental incision. Chin implants are  manufactured in a variety of sizes and shapes which is best determined at the time of consultation and examination, since the chin is a three-dimensional structure. Your images illustrate a 'smaller lower jaw' with an 'overbite/overjet'. The lower jaw is 'behind the upper jaw/incisors' so that you have a couple of options. The most ideal, but more extensive would be to re-enter orthodontics/Invisalign and combine it with a lower jaw advancement surgery. The second option is a sliding genioplasty as opposed to an implant. The sliding genioplasty will give you a 'flatter' area under the chin since the 'muscles attached to the chin' extend with this approach; and not with an implant. Liposuction could be considered but is likely not necessary depending upon which option you choose.
