What is my best option to fix asymmetrical lips due to trauma? (photos)

Question

I fell and bit through my lower lip 9 mos. ago, requiring stitches outside and inside. Unfortunately, my lip is now asymmetrical, especially when I smile. This has affected my confidence when talking to people or smiling for pics. I've seen 2 plastic surgeons (around 5-6 mos. after), and both said they didn't feel any scar tissue. I'm told either a kenalog injection or scar revision may help. I'm reluctant about the kenalog due to risk of discoloration/indentation. What are my treatment options?

I believe you will do well with lip augmentation using a dermal filler. I would ask to consider a softer filler with smaller HA particles such as Restylane Silk or Volbella. Consult with an expert level injector for best results. Best wishes, Dr. ALDO

Hello,Thank you for your pictures. It looks like your asymmetrical lip shape is more of a "dynamic" vs. static problem. Hard to tell from your pictures. A scar revision may help. You may have suffered from an injury to the nerves that go to the lip. If this were, the case, you should wait to see how things evolve. Thank you for your question. Fat injections provide the only natural method for potential permanent results. It has the potential to even out your lips, and smile. There are surgical options as well but it is important that your consult with a board certified facial plastic surgeon. Your experience after your trauma will guide the treatment plan as well.

Thank you for sharing your question and photographs.  In the hopes of improving your current asymmetry I would advocate for placement of a hyaluronic acid based dermal filler.  This will cause an increase in your overall lip size but will camouflage the raised area of injury.  After 12 months if your left lower lip does not flatten you can consider a procedure to help reduce the area.  Hope this helps.

That is a well meaning suggestion that a surgeon makes when they don't really understand what you need.  The steroid will kill fat in the lip and make you worse.  I think your lip actually looks amazingly good for your history.  You know your history and that makes you aware but others are likely to accept this difference as normal.  Consider having fillers to help balance the appearance of the lip.  Less is more.

Thank you for your question. Dermal fillers such as Juvederm and Restylane Silk are the most popular choices for symmetrical and full lips. Also, these are hyaluronic acid fillers that can easily be dissolved with an enzyme known as hyaluronidase if you are unhappy with the results. Lip lift is not a good option as it can lead to scarring, which is difficult to treat. I recommend having a consultation with an expert injector to determine the best treatment plan for you. Without a careful examination it is impossible to give specific advise. I have used scar revision procedures, fat transfer, ultrasound and occasionally steroid injections in varying combinations to improve traumatic deformities to lips.

This can be improved with properly placed filler. It will make your lips slightly bigger. I have used filler in cleft lip patients with great success. It is a more difficult injection and takes a more experienced injector.
