Hi, I used eyelid tape for years and created permanent double eyelids. I'm in my 30's now and they haven't changed in years. However, I'm concerned that my creases are too high and unnatural looking. I also have an extra fold in the left eye. Do you think they look unnatural? How would you describe my eyelid fold? How would you improve it if at all? Thanks
Answer: Double Eyelids Ethnically Asian eyelids have a wide variety of creases, and there are various preferences, however, in general, males have relatively low creases (5-6 mm) and yours does look slightly higher than that. In addition, you see to have multiple creases. You would be a good candidate for double eyelid surgery (Asian blepharoplasty). Make sure you choose a surgeon that has significant experience with Asian eyelid anatomy, preferably with training in South Korea, the Mecca of double eyelid surgery. In deciding which type of eyelid would work best, it is important to look at one’s surrounding facial features as well as individual preference to determine what would appear most natural. Eyelid shapes can be divided into three major categories: tapered, parallel, and hybrid shaped which is a combination of the two. Based on your photographs, your upper eyelids appear to be more of a tapered/hybrid shape, where the eyelid appears thinner towards the inner corner of the eye and gradually flares out laterally towards the outer part of the eye. The height of the double eyelid fold determines how much of the upper eyelid is visible. In general, a high double eyelid fold tends to look less natural in Asians. Upon viewing your photos, your upper eyelid shape and size seem appropriate for you. The extra skin folds below the eyelid crease is due to excess, loose skin that may have resulted from using tape for many years which can often stretch out the eyelid skin over time. An Asian Upper Blepharoplasty (ie. Double Eyelid Surgery) would help you establish an upper eyelid crease that is more clearly defined as well as correct the multiple folds of skin. I recommend you see a Board Certified Facial Plastic Surgeon or Plastic Surgeon who is experienced in Asian Upper Blepharoplasty to evaluate your eyes in detail.

The main issue you have however is what I would term an 'untidy fold'. This is different to a triple fold which can be a result of scarring or congenital tethering. While you have a distinct upper most fold, the skin underneath this fold has become loose (possible due to years of tape use) and is forming excess folds underneath your main fold. A formal double eyelid procedure will correct this problem bu reducing the amount of skin excess in the lower part of your eyelid and tidying up the fold lines. Hope this helps and all the best.
