5 months post op, my left breast is more inflated on top than right. What’s going on? (photos)

It is difficult to get a complete evaluation with only three 5 month postoperative photographs. Sometimes the situation really just needs to be seen in person. However, from your photographs, it does appear that your left breast, (the breast on the right of the photograph), is a little higher. Although I can see some asymmetry in your three photographs, overall, your results look good. You certainly do not need to have additional re-visionary surgery at this time. The asymmetries might still improve with time. You could also try to wear a breast band and do downward massages. If the asymmetries do not improve with time and these maneuvers, you could always consider the re-visionary surgery at a later date. Although, you will always have some asymmetries, even with re-visionary surgery, you need to decide if your existing asymmetries bother you enough to undergo additional surgery.

The photographic views are limited but from your history, it appears that your left implant is "riding high". This is not uncommon and requires a simple pocket adjustment to correct. 15% of cosmetic procedures may require a touch up. 
I would replace the implant only if there is also a size disparity.
