I had a septoplasty and turbinate reduction in March 2024 (writing this in October 2024). The surgeon made my nasal cavity so wide that the appearance of my nose has drastically changed. The tip/base of the nose is now much wider and flatter, and it’s causing me a lot of insecurity. Is there hope that a revision surgery could fix this issue and make the nose look closer to the pre-op appearance? I’ve included 2 before and 2 after pictures.

Answer: Revision rhinoplasty You can have hope that you can get the nose that looks and works to your liking. Despite the fact that you had a septoplasty, it is clear that your septum remains significantly deviated, and now with loss of sidewall support and likely internal valve closure with inspiration (you still can't breathe!). You will need more cartilage that is available in your septum, because the surgeon likely also removed cartilage from there. This means harvesting your own rib cartilage of using fresh frozen cadaver rib cartilage. In terms of aesthetics, photo morphing technology allows you and the surgeon to align a vision for your goals.

Answer: Revision rhinoplasty Hello and thank you for your question. Based on your photographs, you may benefit from a tip refining revision rhinoplasty. Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. Cartilage grafts may also be used to help improve tip refinement. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person. Best wishes and good luck. Richard G. Reish, M.D. Harvard-trained plastic surgeon
