I am 27 yrs, old Asian woman. I am much concerned about my dark circles around my eyes(mainly on eyelids). I tried chemical peels and consulted cosmologist, he suggested me some lightening products but nothing worked and they became worse. So now I am planning to get PRP injections on eyelids. I don't know whether it would help or not. I am totally upset with my dark and dull eyes. So please help me to get rid of these charcoal eyelids instantly. Thanks!

Answer: Limitations of PRP - the real truth. Dark circles are very common and occur more frequently in Ethnic patients. The very first step is to determine the CAUSE of dark circles. In most cases this is racial (I have them as well), in some cases, it is due to deep tear troughs, in others a combination of PIH or skin hyperpigmentation due to eczema, and in many cases a combination of factors. Dermal HA fillers are often used in this area to correct volume loss due to deep troughs, my preferred fillers include, Restylane, Belotero and JUVEDERM Volbella. I do place the filler deep- namely on bone, this reduces the chances of a Tyndall effect (blue tinge). Fillers correct only one aspect of dark circles. In many cases combining a low power laser such as Clear and Brilliant or Fraxel 1917, in addition to skin care can help. Other ways to treat dark circles under the eyes include - Treat any skin irritation - namely simplify your skin care regime- use a good no- irritating moisturizer- this can reduce inflammatory changes on the skin. Low dose Q switch lasers, or pico lasers. This can reduce pigment and increase collagen. Microneedling and PRP- this has to be combined with other treatments to be effective. As a stand-alone treatment it is unreliable in giving predictable results. Most often no results once the swelling goes down. Low strength HQ creams – this is used to decrease melanin activity in some cases. Multiple sessions of either Clear + Brilliant or Low Level Fraxel 1550 (I do however believe that Picosure Focus is better than Fraxel in my hands for dark circles- 3-4 sessions needed). Very low strength peels including 6-8% TCA – five to six peels in combination with fading agents. Even with multiple treatment methods, dark circles can only be reduced, and not totally eliminated. Realistic expectations and understanding the cause are crucial in treatment of this problem. Some causes such as constitutional pigmentation cannot be modified, as genetics cannot be changed. In the meantime, yes, PRP would be a good option to rejuvenate and enhance the skin around your eyes, as will as reduce pigmentation. It is a rather fast and easy process, made from your own plasma. Remember, you need to have realistic expectations, and patience until you see results with PRP. Wishing you the best.
