Can CoolSculpting or Kybella cause scar tissue? If so, can it be removed? Or, can the submental fat be removed still? (Photo)

I’m 30 years old and have done several treatments in hopes of eliminating my double chin. None of them have worked. I’m not overweight, it’s hereditary. I’ve tried Kybella (1x), CoolSculpting (1x), and have done submental liposuction 2x (In this order!) I still have a lot of noticeable fat under my chin. The last surgeon said the scar tissue was in the way of him taking the fat out. How can there be scar tissue? is there anything else I can do to get rid of the submental fat?
