I'm Asian. I was born with double eyelids that would disappear when I open my eyes. L I had eyelid surgery in 2013, the method was mini incision and only fat removal. I choose a medium high crease, the suture is around 10mm from the lash line. I feel like my eyes are much rounder now after eyelid surgery, how do I make them less round? Lateral canthoplasty? Ideally I would like my eyes more almond shaped like before and in the last picture. I have a lot of skin on my eyelids.

A 10mm tarsal crease is on the wider side, and that combined with fat removal can make your eyes a bit more hollow and round (compared to your pre-op photo). Sounds like what you like is more of an elongated eye shape. Medial epicanthoplasty and lateral canthal extension can elongate your eyes by extending the inner and outer corners, respectively. Just know that there's a higher potential risk of visible scarring from these procedures compared to the double eyelid surgery. Hope that helps.

Answer: Upper eyelid shape Honeyfosho, I think the shape of your upper lids are quite pleasing. You do have a mild difference between the epicanthal fold of the right upper lid (incomplete) and the left upper lid (complete). I agree with previous posts that doing anything surgical may worsen the eyelid shape. The primary thing that stands out is that your eyebrows appear more masculine- straight without arch laterally. If you cover the eyelids of the other models you like, you're find their eyelids look very much like yours. A trial of botox laterally may raise the brows and change the appearance of your eyelids. What you are searching for may not be obtainable, and you run the risk is further scarring from some of the procedures already described by others. Your eyes are rounder because of the eyelid surgery and further surgery is not without risks. That's my advice after 30 years doing aesthetic surgery. Answer: Revisional double eyelid blepharoplasty The procedure has increased the vertical length of your eyelids and has deflated the soft tissue/fat, creating a more round appearance. I agree with my colleague that a medial epicanthoplasty can improve the aesthetic appearance but the procedure can be unpredictable in terms of outcomes due to the nature of the healing forces in the area. You can consider re-inflating the eyebrow/eyelid fat pad with hyaluronic acid filler as this will soften the round appearance. Similarly coating the bone in the lower eyelid with filler can raise the lower eyelid margin. I would be inclined towards a non surgical approach in the first instance before considering revisional surgery.
