Thank you for your question. During the revision surgery, the pocket on lateral side should be partially closed, while medial side space, if it’s closed should be opened. Moreover, to make inframammary span tighter and the cleavage looking more natural, I would recommend performing fat graft on the medial borders of the implants. However, while your breasts only slightly low, I wouldn’t recommend the breast lift, as it would leave you with visible scar.
Your breasts are only held up by the skin envelope. The weight of the implant plus your breast tissue is causing your skin to stretch and deflect inferior and laterally. You will need smaller implants or reinforce the breasts internally. Your lateral pockets can be tighten to prevent them moving so far to the outside of your breasts. You probably need a lift to help tighten your breast tissue, not implants, so the tissue is positioned well.

Thank you for your question. It is hard to say by just looking on the photo, but it can be caused by the left implant slightly dropping, right breast to be more prone to sagging or the capsule on the left breast might be thicker, which makes the implant going up. If the cause is in the implant...
Your implants are too large and heavy for your tissue and are sagging. they are also not completely settled down in the pocket because of their large size. You need to go smaller.
