I am 4 months post op and I’m not satisfied with my results. My natural nose was longer on the front view. Now my nose feels very short and lifted from the front. Looks almost cut off? I don’t mind the side profile but I don’t like how much nostril show I have in the front and how the tip is well above the base of my nostrils now. Can I de rotate to help this? Thoughts? I feel I will want a revision once I am one year to address these issues

This depends too much on how you look and what your goal is. Ear cartilage is, all thinks considered, a small amount of cartilage. If you are just trying to build up a little, then that could be enough, but if you are trying to build up a substnatial amount, most likely it will not be enough...
The main reasons for general anesthetic are: number one to correct to protect your Airway, and number two to keep you comfortable. Your nose is part of your Airway, and invariably some blood will drip into the throat and this can cause coughing if you're awake. You would need to be more specific...
Have you done any steroid injections? this looks like it's itching for one. Otherwise it's too hard to tell what could be going on without access to your op notes
