I've got a melasma moustache and it drives me nuts. Revlite has helped but it comes back. I've tried Tri luma and hydroquinone on its own. Never tried peels. I have heard picosure is safer than revlite. I'm a very pale girl with dry sensitive skin.

Answer: Melasma Melasma or facial pigmentation is a very difficult condition to treat. In our office, we use a variety of treatments including skin care products (containing retinoic acid, hydroquinone, glycolic acids), sunscreens, chemical peels, and lasers (IPL/BBL, Q-switch, PICO, Fraxel). Over the last few years, we have had great success with the ZO Blue Peel in treating hyperpigmentation. This is a robust in-office procedure that provides dermal and epidermal stimulation allowing for maximal tightening, restoration and rejuvenation of the skin. It also minimizes or eliminates the appearance of melasma [both epidermal and dermal melasma] and facial pigmentation. All patients require 6 weeks of skin conditioning prior to the procedure with the ZO Hydroquinone or Non-Hydroquinone kit. These are advanced techniques so be sure to get an experienced dermatologist or plastic surgeon consultation for recommendations and treatment options.

It is important to have this problem diagnosed and managed by a specialist dermatologist as many laser/ IPL/peel and other topical treatments can make the problem much worse. in our practice the PicoSure is proving promising, often used in conjunction with newer topical treatments available to (and understood by) experts in the the treatment of pigmentary disorders. One of our specialists has studied extensively overseas to become an expert in this field - she is using the PicoSure in combination with other medications and our previously frustrated patients are thrilled with the results. Choose your practitioner wisely - any laser is only as good as the skill of its operator.

I still prefer the use of bleaching cream, vitamin A, gentle peels and low dose 1064 laser. Having said that, for resistant cases of melasma, Picosure has helped.
