It doesn't look like a problem to me. There are a few considerations:
(1) What do you need done? It is possible we could correct the remaining issues without opening the columella.
(2) When was your last surgery?
(3) I would put you in our hyperbaric oxygen chamber before and after surgery to prevent any problems.

It looks as though you haven't had good luck in selecting a surgeon. That's more important than the blood supply issue. I don't ever remember seeing a nose where that was a problem, certainly not an insurmountable problem. Worry more about finding the right doctor this time!

Since you've had an issue with poor blood flow, further surgery would be risky. In you narrative it's not clear what you dislike about your nose. Please consider re-asking your question with photographs of your current nasal appearance. Depending on your examination, if you have asymmetries or indentations, microdroplet Silikon-1000 injections may be considered. Since the size of a Silikon-1000 molecule is smaller than a red blood cell, this would never lead to vascular occlusion or necrosis.

Based on your history alone, further surgery carries a high risk of vascular compromise to the skin. Your case requires in depth discussion in a consultation. But the simple answer, is "yes", you should be careful.
