There’s no definitive criteria for defining who has gynecomastia and who doesn’t. In your case, I think most providers would agree that you do not have any signs of obvious gynecomastia. The male chest will have a very different appearance with a Arms in a down position compared to when your arms are extended over your head. Extending your arms towards the soft tissues on display and is an excellent way to assess Excess of glandular tissue. There may be some fullness that appears in your chest when you put your arms in a down position, which is the accumulation of skin redundancy, allowing for the range of motion of your shoulder joints. With age we lose skin, elasticity and skin redundancy typically increases, causing men to get fullness in their chest with age. This does not mean they develop gynecomastia. Best, Mats Hagstrom, MD
