I have the typical large and flat asian nose that I feel is not in harmony with the rest of my face. After doing some research I am planning to get bridge augmentation, tip plasty, and alar base reduction. I think I have alar overhanging and a retracted columella. What procedure can take care of this, or would an alar base reduction be enough? Is it commonly done in the US on ethnic noses?

Answer: Asian Rhinoplasty In your situation, a limited procedure such as an alar base reduction, would not be enough to address your concerns. Typically in Asian Rhinoplasty, the bridge is raised, the tip is reshaped/projected and the alar base is narrowed. I feel you would need all of these maneuvers to achieve an optimal result. Please seek a board certified surgeon with sufficient experience in Asian Rhinoplasty

Answer: Asian Rhinoplasty A typical Asian rhinoplasty usually involves dorsal augmentation (either with an implant or autologous tissue), a tip plasty to increase tip projection and in your case also lengthen the tip to decrease the appearance of a "retracted columella". A tip-plasty is typically sufficient to refine your nasal tip, but you may also benefit from reducing the alar base which is quite common in Asian rhinoplasty as well. An in-person consultation would be necessary to render you a more accurate assessment. Goretti H. Taghva MD FACS
