I currently have 345cc saline implants and will be replacing them. Doctor recommended 375-400cc moderate silicone implants. I want to look a bit fuller on top and for my breast to be a little closer together. I want to look fuller but not super fake. Is this a good choice? Also, would you recommend going through nipple or under breast. He recommends under breast.
Answer: Implant Exchange I think you'll be happy switching from saline to silicone implants. What size to go with is a very important consideration. For patients that want to see a noticeable difference in size from their existing implants, I usually recommend increasing the volume by at least 100cc. I will typically utilize the existing scar from the original surgery to replace implants- whether it is infra-mammary fold, peri-areolar or trans-axillary. This way you will not end up with new scars. Your surgeon should perform a thorough analysis of your body measurements to help in choosing the proper size and profile of implant that will give you the optimal results. If you are still unsure about their recommendations, ask your surgeon to explain their rationale. They should be happy to provide this information. Answer: Should I go with 375 or 400cc silicone moderate implants? Thank you for the question and pictures. You will find that online consultants will not be able to provide you with specific advice in regards to selection of specific breast implant size/profile. Ultimately, careful communication of your goals will be critical. Generally speaking, the best online advice I can give to ladies who are considering revisionary breast augmentation surgery (regarding breast implant size/profile selection) is: 1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. Ask to see lots of examples of his/her work. 2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining breast implant size/type/profile will most likely help achieve your goals. In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. For example, I have found that the use of words such as “natural” or "C or D cup” or "fuller but not superfake" etc means different things to different people and therefore prove unhelpful. Also, as you know, cup size varies depending on who makes the bra; therefore, discussing desired cup size may also be inaccurate. 3. Once you feel you have communicated your goals clearly, allow your plastic surgeon to use his/her years of experience/judgment to choose the breast implant size/profile that will best meet your goals. Again, in my practice, this decision is usually made during surgery ( after the use of temporary intraoperative sizers). Viewing the patient's chest wall in the upright and supine positions, with temporary sizes in place, help select the best breast implant size/profile for the specific patient. Answer: Breast Augmentation First and foremost, I would use the same incision that you had before to change the implants so you dont have additional scars. As for the size, the difference between 375 and 400 is minimal and therefore I would recommend going with 400 so you get the fuller look that you are looking for.

That being said, the volume that a saline implant occupies per cc is more than what a silicone implant occupies per cc. In other words, if you were to have an augmentation of 350 saline on one side and 350 silicone on the other, the saline side would appear bigger. Thus, you may wish to go over 400 to give yourself more superior fullness and if your goal is to have you breasts appear a little closer then a moderate plus implant would be my choice. If your surgeon has a range of sizers, you can have him show you a 375 and 400 side by side and you will notice there is not a big difference. Thus, I would suggest you go to at least 400 if not a little more. For a simple exchange with slightly larger implants, you should be able to use your previous incision, even if it is around the nipple. If you had armpit incisions, then under the breast may be better. Best of luck with your surgery!
