I am an 18 year old female. I have large prominent eyelids. I have hollowed eyes so it makes my crease high. Would fat grafting be an option to help my eyes less prominent and lower my eyelid crease? I don’t like how my eyes look tired from how hollow they are. The hollowness in my eyes might be because of genetics but no one else in my family has large eyelids like I do with a high crease. I want my eyelids to be smaller with a lower crease, what surgery would be recommended?
Answer: Eyelids From your photos, my judgement is that the appearance you want can be most easily achieved by raising the position of both upper eyelids (for lower crease and more awake open eyes) and lower eyelids. Resting eyelid position on your globe (eyeball) determines how much and what part of your eyes your palpebral aperture shows. You want both upper and lower eyelids elevated a couple of millimeters. We do this by levator advancement of upper lids and Piezo canthopexy of lower lids.

Answer: Grafted fat does not fix this. You have bilateral central levator disinsertion ptosis with a compensatory eye brow elevation. The fix is anterior levator resection ptosis surgery to raise the upper eyelid. This should be done in conjunction with crease lowering and mobilizing the pre-aponeurotic fat into the upper eyelid fold. That requires upper eyelid surgery by a specialist. There is no substitute for a detailed in person assessment.

Answer: Smaller cheek bones, possibly eyelid ptosis These photos do not show enough of your face to evaluate size of your midface and periorbital area. It appears that you have small, flat cheeks, what makes your eyes look larger? It is also very likely that your high upper eyelid creases are due to eyelid ptosis? Augmentation of the cheeks in your age is primarily done with cheek implants in my practice but fat grafting is an option as well. Correction of the eyelid ptosis can be done in the same surgery. Answer: Blepharoplasty Fat grafting would have no effect on your super tarsal crease. You appear to have upper eyelid ptosis I would recommend consultation with an oculoplastic surgeon
