Question

I've been wanting a more round lifted butt for awhile now. I grew up playing sports and have remained pretty active, but my butt isn't what I would like. I was wanting a bbl as I've read they come with less risk of complications, but I'm not sure I would have enough fat to transfer to make a real difference, so I've been considering butt implants. I would love a professionals opinion, thank you! My measurements are 36-25-37 and I'm 22 years old

Hello and Great Question.  I appreciate the time you are spending researching this procedure.  My name is Dr. Matthew J. Nykiel.  I am a Board Certified Plastic Surgeon, a Fellowship Trained Cosmetic Surgeon, and a Butt Augmentation Specialist.

Based on your photos, you do not have enough fat alone to meet your goals.  A combination of liposuction to help bring in the waist with butt implant to round and increase the volume of the butt.  You can use the fat that was sucked out to help finesse and fine tune sculpt the butt.

Either could work.  If you are going to pursue the fat transfer, make sure you select a board certified plastic surgeon who specializes in thinner patients.

Thank you for your question and for posting appropriate photos. You do seem to be a good candidate for gluteal implants as you don't appear to have enough fat to get the results you want. You might consider a combo of BBL Fat Transfer and Gluteal Implants to get the most natural results. Either way, make sure you have a consultation with a board certified plastic surgeon with expertise in these areas.

There are a few things to consider when deciding between fat injections and implants. First of all does the patient have enough fat for a BBL? You are in no way shape or form fat, but there is a lot extra fat hidden in places like the inner thighs that your surgeon could use. Will it be enough to make you happy with the size of your butt? Maybe not if you want it very very large. That being said butt implants are a lot more complicated than breast implants. They have high infection rates and may need to be removed. The implants may also shift and cause the buttocks to look asymmetrical. To sum it up there are risks to every procedure, along with pluses and minuses for each individual patient.

Thanks for posting, the photos are also quite helpful. You are quite thin, so while some liposuction and fat transfer may be possible, it will likely not be enough for you to notice a significant difference (certainly not like your "wish" photos). Buttock augmentation with implants would be needed to give you a significant volume change. Be sure to find a board certifed plastic surgeon with buttock implant experience, as these surgeries are often doen incorrectly. Also, the recovery is a couple weeks, with limited sitting, and the pain is significant so you will need to be prepared for that.

If the type of which pictures you were showing is what you seriously want to achieve that I don't think you have a choice but to go with implants with or without a Brazilian at the same time.

The majority of plastic surgeons do not perform with your implant surgery.

There's a handful of people who have expertise with this procedure.

I would definitely go to someone who has done many many of these procedures.

Myself personally even though I don't perform gluteal implants but have a lot of experience with fat transfer I would be hesitant to recommend gluteal implants.

I do have somewhat negative reputation for complications, unnatural aesthetic outcomes and other drawbacks.

But it's not to say it is necessarily A bad procedure.

My recommendation would be to have a BBL to start with by someone who has an extensive amount of experience.

Even though your slim if that was harvested from multiple areas you would have enough fat to have a very nice change in contour.

To enhance the buttocks you generally always want to perform liposuction of the waist and hip areas anyway.

Consult with several board-certified plastic surgeon is to perform Brazilians as well as plastic surgeons who have extensive experience with gluteal implants.

Look at implications of having gluteal implants not only in terms of aesthetic results, complications but also other issues such as discomfort limiting physical activities, numbness etc.

For an excessive profile are very dramatic results in plans or a combination of both will probably be needed.

Ask yourself do you really need that much projection.

You have an incredible figure to start with and a somewhat more subtle enhancement would go a long ways in my opinion.

Both free fat transfer (Brazilian Butt Lift) and buttock implants are options to enhance the buttock.  In your case, I believe that fat transfer, BBL, would give you a nice result and carry less risks than buttock implants.  Given that you are on the thin side, I don't believe your wish photos are realistic with either BBL or implants.  But, I believe you would see a nice improvement with a BBL, alone.   Even buttock implants my not be able to fully achieve your wish photo, and would carry a higher risk of long term complications and potential for post op pain/discomfort.   Seek out a board certified plastic surgeon who has experience in both BBL and buttock implant surgery.  Best wishes, Dr. Lepore.
