My mentolabial fold in particular is my biggest insecurity. I've often gotten comments about my mouth area looking like Homer Simpson. My chin area looks like it's dark from hyperpigmentation, but it's a shadow from my protruding lips. How do I fix this area? My chin is rather long, would adding filler/implant into the fold elongate it more? Would lip fillers exaggerate the fold more? Is a chin reduction necessary or would it ruin my face?

The problem lies in the short and angled back chin with a deeper labiomental fold. Fillers in the lip or chin reduction will not improve the problem. Bringing the chin bone forward and up (vertically shorten it) will improve your profile and give the opportunity at the same time to help make the labiomental fold less noticeable.
