I had my rhinoplasty 6 mos ago. My Dr. harvested cartilage from Both ears to build up nose bridge. Im on my 7th month and everything is settled already but i'm kind of unsatisfied with my bridge height i want it to be a bit higher and worried that this might even go back to the original height as time goes on. What are my options? Do goretex lasts? How many years?

Answer: Rhinoplasty results after bridge grafts? Rhinoplasty results after bridge grafts? Wait another 6 months and then see a very experienced revision rhinoplasty surgeon. I do not like foreign materials in the nose--there are better options.

Answer: Bridge height AS edema subsides further (up to a year or a little longer sometimes), the relationship of the height to the width will change and you may be more satisfied. You can see none of my colleagues recommend any foreign material in your nose. I agree with this principle. Experienced rhinoplasty surgeons who do secondary work make a good living removing silicone, Gortex, Supramid, Porex, etc. Can you believe ivory was once used for this purpose???

If you ar unhappy with the cartilage raise of your dorsum I give you 2 choices: -if you need a slight raise and some camouflage then the superficial temporalis fascia with its precious millefeuille collagen multilayer is the best option, adds smoothening effect and volume -if you need a substantial height increase then a revision is bound, to fully re-open, extract the graft, add a second layer (rib? septum? if no ear is available) and re-fix in place (probably adding temporal fascial to hide it) Do not either accept a mini-revision or touch up, has to be full dorsal revision or nothing.

Building up the bridge requires grafting material. If it is only 1mm of augmentation, I use rib perichondrium (soft tissue). For a small augmentation, septal cartilage from the nose is great. For significant height, I recommend rib cartilage. I carve a solid graft. I do not use synthetic implants in the nose due to the lifetime risk of infection and extrusion (breaking through the skin). Ear cartilage is challenging to use on the bridge due to its curved shape. After a previous nose surgery, I would wait a year so the skin can recover its blood supply. Safety comes first.
