I appreciate your question. It is often the case that the changes to the nose which you are describing tend to happen after double jaw surgery. If it is looking wider, upturned, and showing more of the nostril, these don't usually improve over the years. A rhinoplasty would be able to narrow the base, reduce nostril show, adjust the tip, and reorder balance after jaw surgery. Due to an upward rotatation, your philtrum is likely looking longer. Often, fixing the nose is all that is needed, in which case a lip lift is not required.

Thank you for your question. For clarification purposes, I undestand double jaw surgery as advancement of the maxilla (upper jaw) and mandible (lower jaw). Certainly a maxillary advancement requires moving the base on which the septum of the nose sits. This certainly can alter the appearance and shape of the nose. I think there is not a predictable pattern of exactly how the nose changes after a maxilla advancement. But it could lead to nasal or septal deformities that are improved by rhinoplasty. I would need to see photographs or ideally be able to examine the patient in person to know for sure

During double jaw surgery one of the main support mechanisms to your nose, the septum, is disarticulated or separated from your cheek bone (maxilla). Once it is separated, the tip loses support and as a result will tend to widen and rotate upwards, shortening and flattening the nose. The philtrum appears longer due to a lack of support along the bottom or caudal part of your septum and nose. Structure and support must be added back to the nose to restore the natural shape and length. The best option for this support tends to be cartilage (rib cartilage) from your own body, and in an experienced surgeon's hands you can achieve a nasal shape even prettier than your original.
