I'd give it 5/10 and want a 9/10, but it seems like I could end up with a 3/10. Is there a physician who could confidently do a 4th rhinoplasty knowing it would look better? Any referrals? Would love to get rid of the sausage nose look, retracted alar, hanging columella, shall I go on?

Your columella appears to be a bit low with a long nasal appearance. This may be reparable, but an examination would be necessary to determine what surgical maneuvers might be best to address your concerns. Thank you. Dr Joseph

Based on your photos, one nostril seems to collapse with deep nasal breathing. Depending on your condition and examination, you may benefit from a cartilage graft to support your nasal tip. The feasibility of this would require an examination. Hope this helps. Dr Joseph
