Removal of septal extension graft and columella strut?

It is unusual to have both septal extension graft and columella strut, which suggests that your surgeon tried very hard to support the tip. It sounds like the Gore-Tex implant is building the bridge projection. Removing the tip support in most instances leads to a decrease in projection, and the balance between the bridge and tip will depend on how much is removed from the bridge. If the lower part of the septal cartilage is intact, a degree of rotation of the tip can produce some support, but would mean a shorter nose. In general, it would be preferable to identify in what way the cartilage creates problems, to be able to define the solutions.

While a full review of your operative note and a careful physical exam is needed to give you the best answer, my expectation is that you would lose a good bit of tip support with removal of these grafts. This can lead to loss of tip projection, a change in the overall rotation of the tip and a pollybeak deformity
