You can make alterations to the columella during revision rhinoplasty. But we would have to see other views of the nose to determine exactly what to do. At any rate, the inside of the nose can be worked on at the same time. Be sure to see a rhinoplasty expert for consultation.

Hello and thank you for your question. Based on your photograph, you may benefit from a revision rhinoplasty with nasal tip deprojection. This can produce a more natural looking result. A columellar onlay graft could also be placed at the same time. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

Much more information is needed, such as a full set of facial photographs from all angles of your current nose, and specific issues of what you're trying to accomplish. A copy of the operative report would also be helpful. If you're having breathing issues that's going to require a in-person examination to document that for medical necessity to bill your medical insurance. Both cosmetic and functional nasal surgery can be performed simultaneously, but the revision rhinoplasty is far more difficult than the functional surgery itself. Look for a rhinoplasty specialist for best results.
