In some situations, the wall can return to its position with very little damage, however, I can see some excess of skin and fat from the pictures. based on your photos, you look like a good candidate for a TT. Full tummy tuck should tighten your abdominal muscles and remove the excess skin. Flanks liposuction would give you a better shape. I would suggest you to consult a board-certified plastic surgeon because only after a thorough examination you will get more information and recommendations.

It will be mandatory to lose some weight in order to create a more balanced body.
Your incision is very high, the abdominal flap is thick and also the mons pubic area and definitely you have some excess of skin as the result of the pregnancy.
So after losing some weight (6-8 lbs) you will be candidate for liposuction all over abdomen, waist, mons reduction (lipectomy), lower the incision, remove some excess of abdominal skin.

Try weight loss first. If then you still have excess fat, liposuction may work. If you have excess skin, you will need a revision. However, avoid any revisions until you have had all the children you want.

An examination is always best, but your should see an improvement in the abdominal contour with weight loss. Once you have achieved you goal weight if you still have concerns then you can consider liposuction. A revision to your tummy tuck would not be needed unless the muscles have separated or there is excess skin.

It’s had to tell from only your photos, but you might be a candidate for liposuction. Whether or not you would require a revision abdominoplasty would require a physical exam.

The fact that your abdomen is bulging despite being at a lower weight should tell you that the changes to your body or not weight related.
Pregnancy related changes are generally abdominal skin laxity and muscle separation.
Skin laxity can be differentiated by pulling the skin tight. You can feel your underlying muscle ball but gently pressing against your skin until you feel the under lying muscle.
A Straight muscle wall should be a bee line between your sternum and your pubic bone.
To get a tight flat abdomen you will need a tummy tuck most likely including muscle tightening.
You should be quite certain But you’re not going to have more children before having a second tummy tuck.
I suggest consulting with board-certified plastic surgeons in your community with the surgeon who did your first procedure.
