Would a second BBL or butt implants get me the best result and do I need a thigh lift?

I had a BBL a year ago. I'm contemplating a second bbl or butt implants, I'd like to know which can give me the results I want, which is a heart shape butt. I feel my butt still looks squarish. The fat used for my bbl was only posterior. NOT 360. My Dr. said my abdomen was too loose to do lipo without a tummy tuck. This time I do want a TT, abdominal lipo with fat transfer to butt or implants. Below are 4 pics. 1 showing arrows on areas I feel need more volume. 1 of my abdomen and 2 wish pics. Also I have loose saggy skin right under my butt, what is the best way to fix this - do I need a thigh lift?

You are in good company with many others after BBL. Going through round two BBL is likely to only give you more of the same without providing additional shape or fullness, especially in the lower half of the buttock, where the fat tends to survive relatively poor. The only more reliable, predictable and permanent way to achieve your desired outcome is with buttock implants. You may in fact benefit from additional body contouring of your front half by way of a tummy tuck with liposuction of the flanks.  Lastly a posterior thigh lift/lower buttock tuck may or may not benefit the area you describe as “saggy skin right under your butt”. The BBL results are highly surgeon dependent.  Some board certified surgeons can do almost anything and change the shape of anything.  You have to be creative.  There are some anatomic limits, but, for the most part, dramatic transformations can be achieved. Have the lipo and BBL first; three to six months later, have the TT.

based on your photos, you look like a good candidate for a BBL. Liposuction of your abdomen, back and flanks should give you more curves and hourglass shape and fat transfer to buttocks should give you more projection. I would suggest you to consult a board-certified plastic surgeon because only after a thorough examination you will get more information and recommendations.

Between whatever fat you may have left in your back and the fat that can be taken from the abdomen during a tummy tuck, you can fill in the hip dips and get that heart shaped butt you want! it seems like you would be a good candidate for a second or revision BBL pay on adding a volume more laterally would help quite a bit. Making your torso especially your back and armpit area smaller would help balance your appearance considering your somewhat thin thighs. I would keep the overall max volume proportionate to your thigh size. Your which pictures have much larger thighs and can handle the gluteal volume in those pictures. Excessive gluteal volume doesn’t balance having thin thighs because these two generally never go together in natural body shapes.I’m not sure why your surgeon was apprehensive about doing liposuction of your abdomen. This does however leave a virgin area with great fat to use for your second BBL. A tummy tuck would look great on you as well.Our practice has been devoted exclusively to Liposuction and fed transfer procedures for the last decade.
