I am heartbroken. I had a BBL almost 2 years ago, April 2018. I specified multiple times I wanted a natural butt and not very big. I mostly wanted my love handles removed and a small fat transfer. I feel like it gets bigger and bigger as time goes on. I feel self conscious wearing bathing suits now because it obviously looks fake. What procedures would I need to help this look more natural, if anything at all? Without me getting further botched? Thank you.

Follow up with the board certified PS who performed your lipo and BBL to determine the extent of the revision that is necessary. A larger butt than desired after a BBL can be liposuctioned to improve contours.  The revision implies that scar tissue will make the surgery much more difficult on the surgeon.  This needs to be done very carefully to avoid skin sagging. This can be tricky as skin and tissue distensibility may preclude any further improvement in shape or dimension with revisional attempts.

The last six months I am getting more patients seeking revision of the buttocks.  In overall people want a smaller more natural appearing products.  My approach to the revisions of the buttocks involve both liposuction and fat injection.  The goal of the procedure is to match the buttocks  to the person's frame.  If the buttocks matches the frame it looks natural, if the buttocks doesn't match the frame it looks fake.  Trying to put a cookie-cutter fat transfer where one buttocks fits everyone doesn't work.The other thing you said about the buttocks is getting bigger over time is true.  Remember when you do a fat transfer you've taken the problem area from the stomach and love handles and those same problem area fat cells have been moved to the buttocks and hips so the buttocks and hips becomes the new problem area where you gain weight first and lose weight last.  If you try to make the buttocks to big it will get bigger over time.  If the buttocks matches the rest the body you should keep your look over time.

It is certainly possible to remove some volume from your butt using a liposuction technique. The revision will be trickier than the first surgery because you now may have some scar tissue that needs to be accounted for. Make sure you get back in for a consult and examination with either your original doctor or someone else who has a lot of experience with BBL revision.The optimal BBL outcomes are achieved with: 1) use of VASER liposuction to harvest the fat, 2) a closed looped system such as Puregraft to process the fat, and 3) strategic micro-packet transplant technique. These three processes have demonstrated over 95% fat graft take. This means that your results will be stable as long as you maintain your weight. As an official VASER trainer, I am routinely referred unhappy BBL patients. One of the most common reasons for disgruntled BBL patients is that patients are not able to accurately communicate their desired buttocks goals to their surgeons.  A buttocks assessment tool can be an invaluable tool to communicate to your surgeon the exact buttocks size and shape you are desiring. I suggest following up with your provider. He or she is responsible for the outcome of your procedure. It’s possible to do liposuction and remove fat from your buttocks.You may not be able to obtain your wish picture either from the initial BBL or a revision but you can certainly have some of the volume removed.Fat transfer is better at giving defuse overall volume then it is to reshape. Reshaping can be done and it’s easier on patients who have certain characteristics like tight smooth skin.It should not be all that challenging to remove enough fat to get you to where you wanted to be at least in regards to size.Your provider is the first person to talk to.I would not call your results botched. It was simply done a little too aggressively. Considering that most patients wish they had more volume or retained more fat most plastic surgeons will fat graft on the heavy-handed side. That is not an excuse for not listening to each individual’s patients needs and wishes.Fat transfer involves a substantial amount of prediction because we always graft more fat than will survive long-term.Follow up with your provider. Thank you for sharing your photos. The non-surgical recommendation is weight loss. Oftentimes, 10 lbs can make a huge difference for patients who feel that their results are too dramatic The surgical option is obviously to remove the fat. However, make sure you consult with a board certified plastic surgeon with expertise in BBL revision surgery. Excess scar tissue can increase the risk of irregularities.
