It appears that the edges around the scar are raised. The treatment of this would largely be dependent upon the type of filler that was used. If it is a temporary filler, perhaps it could be dissolved. Sometimes light electrodesiccation around the perimeter of the scar can lead to a less noticeable appearance. If you were lucky enough to have an HA filler, then dissolving the bumpy surrounding portion should be fairly easy. if you had a semipermanent or permanent filler, not so easy. But, I have had gratifying results using varying combinations, concentrations and amounts of intralesional anti-inflammatory injections to flatten the filler related adverse reactions. Once this is accomplished subcision (which would have been my first choice of treatment for this scar) can be used to elevate and smooth the indented portion of the scar. You would be wise to make sure to consult with a board certified cosmetic dermatologist scar treatment expert. Hope this helps and best of luck.

It is hard to tell from this photograph what needs to be done. I would seek a consultation from a board certified plastic surgeon to find her best options. There are several laser sick and be used on scars that may soften it. There is also the possibility he may need a scar revision.

For indented or atrophic acne scars, I usually first recommend subscision, to break up and loosen the bound- down scars. I would only consider injecting dermal fillers into the scar after the subscision, or else the fillers may make the scar more noticeable.
