For Botox, I mix each 100 unit bottle with 2 ml of saline so each of my injection doses is 2.5 units. I like to start with 2.5 units per side. I get a better result when the Botox is injected at the insertion point of the muscle near the corner of the mouth. If a second injection is required, I place it lower along the border of the mandible.

Prior to any cosmetic treatment with botulinum toxin evaluation of the muscular anatomy should be performed by a trained provider as different facial anatomy require variation in dosing or any area. The majority of patients benefit from 2-5 units of Botox to each DAO which will produce a nice subtle lift to the corners of the mouth as well as can help the appearance minimally of the marionette lines. For maximum benefit I often combine Botox with hyaluronic acids fillers to turn the corners of the mouth and replace the lost volume in the marionette area.

I find that many patients who are interested in Botox injections here can also often benefit from filler injections (with Juvederm or Restylane) to help replace volume loss - this can often provide more dramatic and longer-lasting improvement than Botox alone. For treatment of the DAO if the injection is done correctly it only takes 4 units injected into the insertion of the muscle into the mandible
