I have small nostrils, but the tissue surrounding my nostrils is very fatty/wide and gives my nose a bottom-heavy appearance. I'm considering getting a rhinoplasty that would refine my tip and I'm very worried that my alae would look very harsh and noticeably wide if I don't get them reduced. However, I don't want my nostrils any smaller, and whenever I try to research alar flare reduction, it mentions nostril reduction. I'm not sure what my options are

Based on your photo, it seems as though most of the left side of the lower 2/3 of your face is a tad higher than your right. The corner of your mouth is higher on your left side and so is your left nostril. In my opinion, this is normal facial asymmetry and I am not aware of any means to change...
When you say "developing", I assume you are actively noticing a change in your nose. If that's the case, it could be scar tissue setting into the supratip region resulting in fullness to this area. If this is the case, see your surgeon, as steroid injections may be warranted to halt and reverse...
