My implant scar is below the chin bone. I didn't want to look like Jay Leno so the doc said he stretched the pocket with a large implant and then put in a medium sized one. I assume he shaped it because I asked for a more pointy chin. Anyway, I can feel where the implant has drifted up and above the tip of my chin bone. I'm also interested in having it redone, but not a larger shape. So how or why wouldn't this implant be secured better with a screw or something?

Your chin seems to have better projection then the person in your example picture.
I suggest having a few in person consultations or maybe follow up with the surgeon who did your buccal fat removal.

Either follow up with your last surgeon or have an in person consultation with another cranial facial surgeon. Preferably somebody who works with more complex pediatric craniofacial cases.
