I would likely recommend facial liposuction to remove the submental fat and a chin implant to improve the projection of your chin and balance your facial profile. These procedures could be performed at the same time with a recovery period of about one week. If skin laxity is also a concern, which I would be able to determine from an in-person assessment, we might add a procedure like RF microneedling that would stimulate collagen production and firm up the skin from within. I encourage you to schedule a consultation with an experienced, board-certified facial plastic surgeon for further assessment and treatment recommendations.
I find that VASER liposuction in combination with Renuvion or ThermiRF are best for contouring the chin in those who are candidates, but Kybella can be used as an alternative and gets better results when combined with Thermage/Ultherapy/Exilis, TruSculpt iD, or Coolsculpting. In order to sharpen up your neck and jaw region I would recommend a combination of procedures. First I would suggest liposuction to the neck area to get rid of some of the excess fat you are visualizing from your frontal and profile view. Next I would recommend augmenting your chin with either a fat transfer or a chin implant. This can be decided in office based on how sharp we want the chin to be and how much projection is necessary for your particular case. The two procedures together will provide you with a really nice and natural result that brings more balance to your facial structure. Going forward, I would recommend you to complete multiple in office consultations with board certified experts to discuss your ideal aesthetic goals. With submental fat you'd be an excellent candidate for neck liposuction. VASER ultrasound is a nice method to help with a smoother result, skin tightening, and a quicker recovery.
The photographs demonstrate significant fat deposits in the neck located both above and below the platysma muscle in the neck. There's also a recessive chin profile president. Consider a chin implant to augment the chin forward. A surgical neck lift procedure can accomplish removal of fat deposits below the platysma muscle and liposuction can accomplish removal of the fat deposits above the muscle. A platysma plasty will also be required to significantly improve the jawline. No skin removal is necessary on patients less than 45 years of age. Both procedures can be performed at the same time which allows for one anesthesia and one recovery.
It appears in the profile view that your chin is vertically short with a deep mentolabial fold. I would recommend a lengthening genioplasty with simultaneous submental liposuction. This will improve the overall facial balance.
