I am 55 years old female I had undergone upper and lower blephoraplasty 3months ago my lower lid in both eyes are retracted, please advise when and what kind of revision surgery should I make do i need spacer graft or will a cantopexy resolve my problem Thank you
Answer: First let me say that I am so sorry that you had an unsatisfactory eyelid surgery. Many will meaning surgeons may offer you canthal surgery. This surgery will not repair your issues. It will use up lower eyelid resources and compromise what can be done with a subsequent definitive repair. These eyelids need to be lengthened both horizontally and vertically. Canthal surgery even with a spacer graft will not satisfactorily address the compromised lower eyelids. I address this issue by vertically lifting the cheek over an ePTFE rim implant that is fixed to the bone with titanium micro screws. This material functions as a felting material to permanently hold this advanced cheek soft tissue vertically upward. This replaced the eyelid tissue removed by your surgeon without resorting to an unsightly skin graft. I also use a spacer graft to shape the lower eyelid margin and put the canthus back together but with the cheek lifted, this can be done without tension. Finally, there is absolutely no substitute for a detailed, in person physical examination to precisely assess your eyelids. A Zoom call is no substitute for this in person assessment. Don't be fooled by surgeons who cut corners. You need a definitive repair to restore your appearance. Don't get talked into compromise surgery that will not accomplish these. Your eyes can be your best feature.
