Congratulations on what appears to be a good gynecomastia repair.  Without pre op pictures, it is difficult to understand what was needed in this surgery, and it would also be helpful to know if you got just gland excision and/or liposuction as well.In either case, it does appear that the intra areolar scar is showing mismatched edges.  This may lead to a slightly indented scar.  However, due to the dark color of your nipple areolar complex, the scar should be very difficult to notice.  Allow the scars to heal for a minimum of three months before making a decision as to its visibility and acceptability.

Time to massage! It pretty typical to the scar to tether in gynecomastia surgery. You need to massage, massage, massage. Grab the scar between your index and thumb and pull it away from your body. It will feel "stuck", which it is, stuck on the internal scar tissue. 3 minutes, twice a day. You should be able to roll the skin away from the scar, like the skin on the back of your hand. These scar tethers, when addressed early, release 90% of the time. Don't expect much change for the next 2-3 weeks, and suddenly it will improve dramatically, typically with the big imrpovements coming at 6 weeks and after. For the other 10%, I use kenalog injections or scar subcision, both performed after numbing and completely non-painful. You can always get rid of it, it is not permanent!

At three weeks postop, there is still a lot of time for the appearance of your chest wall to improve. Generally, swelling dissipates, contours improved, and scars mature over the course of the first several months (and beyond) after gynecomastia surgery.  Of course, every patient heals differently depending on many factors, including exactly what procedure was performed. My best advice at this point: ongoing patience and continued close follow-up with your plastic surgeon.
