i dont really like what the orthodontist did with the camouflage treatment. still have weak chin/jaws, when i smile my mouth looks crooked and the chin tilts to the left. i really think maxillo mandibular advancement would benefit me. what was done. 1 premolar removed + palatal expander and 5 years with braces.Also could undereye area/malar area be improved?

I would recommend dental and skeletal analysis with an orthodontist and Maxillofacial Surgeon. Looks like you may need mandibular advancement after further orthodontic treatment. A sliding advancement genioplasty is also a compromised option.

It would be easier and more effective to either have a sliding genioplasty or a custom jawline implant made to make the lower facial improvements that you seek. At the same time you could have infraorbital-malar implants placed to augment this area as well.

I would see an orthognathic surgeon.  They might suggest to undo what the orthodontist had done. If it was me I would only advance the lower jaw alone. The upper jaw seems to be fine. Perhaps add genioplasty.

the orthodontist often does not allow for an ideal 'facial cosmetic' balance; but often focuses just on getting the bite correct. This is less than ideal but is common in my experience. Rather than going through the significant 'jaw surgeries' you could have a masking approach with augmentation of the jawline/chin and the midface. This is easier for you and minimizes lengthier orthodontic treatment and its effect on the teeth roots. We do this commonly since many persons have orthodontics as a youth and find out that the aging changes are less ideal/cosmetic
