My goal is to have an overall smaller nose ( especially when I smile). What bothers me the most is my tip and how large my nose bridge is. How would communicate this to a doctor?

A well trained board-eligible or board-certified plastic surgeon with specialized training in rhinoplasty should be able to perform an adequate physical exam and listen to your concerns regarding your nose. A plastic surgeon who utilizes 3D imaging should be able to simulate your surgery and further understand your surgical goals.

Accoridng to the visuals you submitted the tip of your nose looks droopy and the bridge looks high which is called pseudohump. The bridge in the picture is somewhere between a hump and a pseudohump which needs to be reduced carefully. When it comes to the tip defintion and refinements; in thick skinned patients it is very difficult to achieve these desires. In the pictures one can not conclude if you have a thick skin or not. Your expressions are quite enough for a nose job surgeon to understand.

The best way to wind up satisfied following your rhinoplasty is to clearly convey your goals to your surgeon.  Most if not all rhinoplasty surgeons use computer imaging in order to help demonstrate the changes to your nose that you both mutually agree upon.  Your surgeon should also convey realistic expectations to you.  For instance, you have very thick nasal tip skin and although you can have a refined and beautiful nasal tip, there will be a limit to how small and petite that tip can be.

Thank you for your question. You are an excellent candidate for rhinoplasty. I think rhinoplasty is one of the most beautifying surgeries that we do as plastic surgeons. A large portion of my practice is dedicated to rhinoplasty surgery. It is important to only see plastic surgeons with extensive experience in rhinoplasty, ie perform at least 6 per month and have performed hundreds of cases. Also, critically evaluate on-line galleries to see if their art form resonates with yours. Rhinoplasty is definitely a grey haired plastic surgeon's sport. Best wishes to you.

This is exactly what is supposed to be discussed at the consultation. Based on limited photos- you appear to be a good candidate for Reduction Rhinoplasty preferably performed through a closed technique. I suggest that you seek an experienced Board Certified PS with a wide gallery of before and after photos.

Good Afternoon! Verbalizing what you want is really the first step as you've done here. Photos should be taken in the rhinoplasty surgeon's office which are standardized and clear. From that point, morphing software to change the appearance of the nose is a great tool for communicating expectations. I use a program called Mirror and it works great to show before and after changes. Now, it's not a guarantee of outcome but it helps to be on the same page. A facial plastic surgeon with experience in rhinoplasty is a great starting point. Hope that helps!

A closed rhinoplasty approach can accomplish shaving down the dorsal hump, refining the nasal tip, and narrowing your bridge line with all of the incisions placed on the inside of the nose. No external incisions  are required, and no painful packing is required either. Thick skin in the tip of the nose is going to prevent refinement in that area, so it's important to have realistic expectations. Digital computer imaging of your nose upon your facial features would also be helpful in the communication process with your surgeon. Rhinoplasty is the most difficult operation to perform correctly in the entire field of cosmetic surgery, so choose your surgeon wisely based on extensive experience
