Age 51, giraffe neck - neck lift vs mini vs Potenza? (Photos)

Question

I’m 51 and have a “giraffe neck”, so I’m interested in a treatment to address the sagging skin. My aesthetician recommended potenza but I wonder if it’s too late for that. Hoping for guidance as to which procedure is best for me.

You have platysmal bands under your chin and marionette lines. Your best choice is not Potenza which is a non invasive skin tightening procedure.  You have platysmal bands which are loose muscle.  I'm afraid you will be wasting your money with Potenza.  You treatment should not be just a neck lift or a mini.  Your proper treatment should be a face and neck lift.

With significant neck sagging you would need a facelift / neck lift with muscle tightening (platysmaplasty). At the same time, I would recommend lasers (Fraxel, CO2, Halo, or Erbium) and microneedling RF (Fractora, Morpheus8, PiXel8, Intensif) to improve the crepey skin and wrinkles. Overall, the neck can be treated with a combination approach:

1. Microneedling and laser treatments for texture
2. Botox, Dysport, Xeomin or Jeuveau to help platysmal bands
3. Intensif, Fractora, PiXel8, or Morpheus8 microneedling RF for crepey skin and lifting
4. Injectable fillers like Belotero, Restylane Silk, Versa, and Juvederm Volbella to improve lines and folds
5. Face/neck lifting if sagging is prominent or premature, with or without platysmaplasty (muscle tightening)
6. Renuvion (J-Plasma), ThermiRF, and InMode FaceTite/AccuTite for internal tightening
7. Ultherapy and Thermage for external tightening
8. Regular Venus Legacy, Exilis Ultra, or InMode Forma treatments with facials to maintain tightness
9. PDO NovaThreads or Silhouette InstaLift cone threads for lifting and long-term collagen production

Thank you for your question. Microneedling is an excellent option for addressing fine lines and mild skin laxity, and you may be a good candidate for this treatment. This non-surgical procedure can rejuvenate the skin to reveal a smoother and more youthful appearance with results that could potentially last for up to a year. If you want more dramatic results that will last for many years, a surgical neck lift might be a better option. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon who can assess your needs on a more individual basis.

Your photos show extensive looseness/sagging of both platysma muscle bands, probable fat accumulation around part of the muscles under your chin, and probable loose skin, all centered in the front of your neck.  Considering the extent of this problem and your age, I would suggest a standard necklift to tighten and lift the muscles, remove any fat, and tighten skin, thus creating a much smoother, more youthful looking neck.  I personally don't think anything short of that would give you as much or as long-lasting improvement.

You do appear to have laxity of the skin of you neck and lower face.  I would say the procedure that would give you the most improvement would be a facelift.  This procedure would tighten the skin of the lower face, along the jawline and the neck.  Non-surgical options might provide some improvement, but not to the extent that surgery would.

The photographs demonstrate loose and inelastic skin in the neck, significant platysma muscle bands, and fat deposits under the chin. The only procedurethat is going to accomplish addressing all these issues is going to be lower face and neck lift with a platysma plasty, liposuction of the fat deposits in the neck, lifting the jowls, tightening loose facial and neck skin, and tightening loose facial and neck muscles.
